A Salt Lake City Birth Records request form must be completed before asking for the microfilms. The request form is available at the US REF desk on the 2nd floor of the Family History Library. No circulation to Family History Centers. Access to these records requires patron show proof that they are an immediate family member (i.e., spouse, parent, brother, sister, or child/grandchild relationship) or have a letter from a client authorizing them to search for their immediate family member. Staff must supervise photocopying of the material. May only photocopy information on direct ancestors. All other information must be crossed out.
