This series contains report forms submitted by physicians or persons in attendance at deaths. According to state statute, "Physicians who attend deceased persons in their last sickness, clergyman who officiate at funerals, justices of the peace who hold inquests, and sextons and undertakers who bury deceased persons, must each keep a registry of the name, age, residence, and time of death of such person" (Utah Rev._Stat._57:2031_(1898)). The submitted forms were used by the clerk to maintain a register of deaths in Tooele County.
